Overview

West Sussex County Council together with Crawley, Horsham & Mid Sussex and Coastal West Sussex Clinical Commissioning Groups are reviewing our five year health and social care commissioning strategy, the Dementia Framework West Sussex 2014-19. 

The review will identify whether we are achieving our outcomes and help us to plan our next steps to ensure people in West Sussex living with dementia and their families are supported to live well.

To help us with the review we would like to hear your views. What you tell us will help us to shape and deliver support in the future.

 

Why We Are Consulting

The purpose of this survey is:

  • To collect information about the care and support available to people affected by dementia.
  • To identify gaps in the provision of services and support.
  • To think about, and inform, how we might do things differently in the future.

The data you provide will be used specifically to inform the County Council's and Clinical Commissioning Group's commissioning plans. The data will be stored securely and confidentiality and anonymity is central to the work. West Sussex County Council owns all data in this instance.

The data will be used to help us carry out our statutory duties and day to day business in the public interest.  We will process your data in accordance with the Data Protection Act, and any subsequent legislation, including the General Data Protection Regulations. 

The information you provide will be kept securely for five years before being appropriately destroyed.  West Sussex County Council is registered as Data Controller (Reg. No. Z6413427). For further details and information about our Data Controller, please see www.westsussex.gov.uk/privacy-policy.

Please be assured no information that personally identifies you will recorded in any report or other documents. You will have an option to withdraw your data within a month following the end date of the Survey.
