Overview

This survey is for parent carers of children and young people with Special Educational Needs and Disabilities (SEND). If you would like to enter our prize draw for a £75 gift voucher please enter your email address at the end of the survey.

Short breaks provide fun, leisure and social activities for children and young people with additional needs and disabilities outside the school day. This enables them to try new experiences, learn new skills, develop confidence and have fun!

The Special Educational Needs and Disabilities  (SEND)  Commissioning Team work with providers to make sure children, young people and their families have access to a good range of short breaks.

This survey is part of shaping short breaks services for the future, working with you as parent carers. This survey is for parent carers only. There is a separate young peoples' survey, which has been co-written with young people with SEND .

If you have more than one child for whom this survey is relevant, please complete a separate survey for each child.

Why We Are Consulting

The West Sussex SEND Commissioning Team are always looking to improve our short breaks and leisure offer for families. Parent carers and families are our partners in developing an offer that meets carers need for a break, and provides activities children and young people want  to take part in.

Access to the right support and leisure activities  is about more than just short breaks; some questions do not relate directly to short breaks but will help us to see the bigger picture of what works for you and your family.
