This survey is for young people aged 16 to 25 with a Special Educational Need or Disability (SEND) . This includes young adults with lots of different additional needs, and people who are on the Autistic Spectrum . It might be more appropriate for a parent, carer or supporter who knows you well to fill the answers in with you, or for you. This is fine. We would like to hear from 16 to 18 year olds as well as young adults over 18, because you will soon be 18 and your views are important for our planning.

The SEND Commissioning Team want to offer more Short Break activities for young people aged 18 to 25 across West Sussex but we want to make sure that they are things that you want to do.

Please help us by completing the questions below. There is a box where you can tell us any ideas that will help us plan the activities.

We are also doing some face to face workshops with young people at their settings ( short breaks, colleges and youth clubs ), and two drop in workshops ( please see details below).

If you would like to take part but feel this survey isn't the best way for you to communicate with us, please get in touch and we'll help you find the best way for you to take part. You could send us photos, pictures or artwork if you like.  Here are some examples of using pictures to give us your views.

Why We Are Consulting

The SEND Commissioning Team in West Sussex County Council have listened to what young people and families tell us, and we understand that you and your families would like more choice of Short Breaks for young adults ( 18 to 25 year olds). We think you as young people are the best people to ask about what you would like and what you want to get out of Short Breaks. Your ideas and comments  will help us to plan and offer the activities young people want, when you want them.
